All existing patients can schedule a new appointment or change an existing appointment directly from their patient portals without the need to reenter their information. For each new patient, please make sure you fill out all the fields correctly in order for the system to schedule an appointment.
If you don't have a middle name, leave the field blank. After you submit the form you will receive an email confirmation at the email address you provided. You will then be able to access your patient portal.
